I answered Yes to a question on the Risk Assessment Form. What do I do now?

One

Step 1

Contact a local public health agency to obtain a TB test.

By answering YES to any question on the Risk Assessment Form, you are a candidate for a tuberculin skin test or IGRA and need to contact a local public health agency to receive a TB test. Please note that you are responsible for the cost of any testing and follow-up required.

Locally, you may contact the Grundy County Health Department located at 1716 Lincoln Street in Trenton for a simple TB skin test on Mondays, Tuesdays, or Wednesdays. You may contact their office with any questions you have by calling (660) 359-4196. Appointments are not required for a TB skin test.

Step 2

Complete the Referral and Authorization to Release Information Form .

The form is located in the NCMC website, under “Student Life,” “Health/Wellness/Safety,” and “Tuberculosis.”

You will need to take it with you to the health agency.

Step 3

Complete all required testing and follow-up in the time span the health agency directed.

Please note that documentation of the TB Risk Assessment Form, documentation of testing completion and any necessary follow-up must be provided to NCMC by mid-term week to avoid holds being placed on your account. It is the student’s responsibility to obtain the necessary documentation, send and confirm the receipt by the Dean of Student Affairs by bringing it to the Alexander Student Center or by mailing to:

All NCMC students are expected to comply with the targeted testing program and any course of action deemed necessary by the local public health agency.
